check availibility button not working

Hello support team,
I am trying to end some customisation for my website in order to put it online for my client and i am facing a little problem with the rooms check availability form. i add pictures to the topic.
When you click on check availability, it always send me on blank screen.
Can you help me please ? Is there any configuration i have to do before be able to use this function?thanks
