Missing Mail Chimp Form on adot theme

      I noticed the Mail Chimp form on the footer as shown in adot-demo 9 was missing in my new installation. I tried fixing by creating the same on my own using …/dashboard>mailchimp>form and got close but not exactly the same.

      Would you please create it to look like demo 9?
