Page Title and header Images disappearing

Recently a few of our pages have lost the background image for the title of the page. All of these pages were using a “Custom Title and Subtitle” with the image selected.
When creating a new page, it will only use the default background image and it will ignore the setting to hide the title. So I have 1 page that has the page name in the header with the default image, and every other page acts differently.
