Become a Teacher Registration Form is No longer appearing

At one point I was able to view the Become A Teacher form on the site for someone interested in becoming an instructor.
Now this form no longer appears. (This form triggers the emails that we need to alert us when someone is interested in becoming an instructor).

When clicking in the area that says “Please Login to send your request to become a teacher a call is made to a USER Login page that calls a [WP_EVENT_LOGIN] because anyone can register. However on this from the Become a Teacher Option does not show.

So I created the Register Now button that calls a from that does show the Become an Instructor check box option. This is tied to the Accounts/register form.

After filling out the initial registration form another Register screen appears.
Once you click on this Register Button after filling out the information a second time, it takes you to the Accounts pages only.
