Form submission problem of “Become a Teacher” form

Hi,
I’m Milton. I’m using Eduma theme to develop a learning site. I’ve already developed the site and hosted at a Live server. The SIte address is digidemmy.vistechsolutions.net. But I’m facing a problem regarding “Become a Teacher” page and the problem is my user cannot submit the “Beacome a Teacher” form because it is not getting submitted and showing no errors and not getting any information at backend.
