How to set up the contact form properly

Hi,
sorry to bother you again on this issue. I have now received the first contact form I sent to myself and I realized that i cannot see the sender’s e-mail address, or add any additional requested information.
ON my homepage: http://www.lessoning.org I have a contact form, there you need to type in the e-mail address and message. However, the message I received was like this:
From: [your-name]
Subject: [your-subject]
Message Body:
Diese Nachricht wurde um 16:44 gesendet.Apparently, I set the contact form wrong at the beginning so that it shows “your-name” instead of the e-mail address of the sender.
Could you help me set up a full contact form with “Name”, “E-mail Address”, etc.? I made a screenshot of my setting right now. But I am not sure if they are correct.
