Quiz result not working say skipping all question

Hi team.
I set the Quiz to Course. but It won’t work.
It never showed the questions then automatically said “Your quiz grade failed. Quiz requirement 80%”
And the results show “Skipped 20” (There is 20 questions in that quiz)
I attached the screenshots.
Please solve this issue as soon as possible. if any upgrade in plugins please provide me i upload on our website.
please this issue solve quickly because website is live.
